I, as a player in a Legacy Park sport program, I agree to do the following:
o Play this game to have fun.
o Work hard to improve my skills.
o Make every reasonable effort to be at all practices and games.
o Pay attention to the coaches and be respectful to them.
o Cheer for my teammates.
o Not yell at my opponents or the umpires or referees.
o Pick up anything I bring to practices or games.
I have read the above Code of Ethics and fully agree to abide by and support it as a player in a Legacy
Park sports program.

Photo Release^*

My submission of this registration form gives permission for Legacy Park to post pictures of the participant on our team website, newsletter or organization Facebook. I give the LP webmaster, Director of Tennis and/or Legacy Park Sports Coordinator permission to post pictures for the purpose of promoting Legacy Park Tennis. Photos may be submitted to the webmaster, director, or coordinator for posting by any member of the Legacy Park Tennis organization. The Legacy Park webmaster, director, and coordinator promise to not post any photo that deems itself inappropriate, and agrees to post only photos that are in good taste and reflect the wholesome sport of tennis.

Medical Release^*

I/We the parent(s) of the aforementioned child permit him/her to participate in all practices and games during the sports season.

I/We assume all risks and hazards incidental to participation, including transportation to and from activities.  I/We do hereby waive, release, absolve, indemnify and agree to hold harmless the Legacy Park Sports League, its sponsors, supervisors, participants, volunteers and coaches for any claim resulting in injury to the above named participant.  I/We do hereby agree to hold harmless Legacy Park Communities, Legacy Park Homeowners Association, Inc. from any responsibility or liability for the health and well being of the participant during participation in the Legacy Park Sports League.

I/We hereby grant permission to the adult managers, coaches or volunteers to obtain medical care from any licensed physician, hospital or medical clinic for the participant if a minor child at such time as either parent or legal guardian cannot be contacted in person or by telephone.

ADULT PARTICIPANT LIABILITY RELEASE AND ACKNOWLEDGMENT


In consideration of the Legacy Park Community Association, Inc. (“Association”) allowing me to use the Association’s property and amenities (“Association Property”) for _____________________ (“Recreational Activity”), and other good and valuable consideration, the receipt and sufficiency are hereby acknowledged, I hereby agree and acknowledge as follows: 

1.	I, the undersigned, understand and acknowledge that participation in the Recreational Activity can be hazardous and I realize that no one should enter into a recreational activity unless he or she is medically able.

2.	I realize that all risk associated with this activity, including but not limited to, falls, contact with other participants, equipment failure, effects of weather and conditions, are assumed solely by me and that the responsibility to ascertain if engaging in this activity might prove harmful to a participant is solely my own. I have examined the condition of the Association Property and warrant that the same are in good working order and condition.

3.	I agree to abide by all Association rules and policies and with customary safe practices, and I release and forever discharge the Association, its members, officers, directors, employees and agents from any and all claims, costs, causes of action, and liabilities for personal injury or death, damage to or destruction of property, or any other consequences thereof which arise from or are in any way related to the use of the Association Property. 

4.	I understand that the Association, its officers, directors, members, agents and employees ("Released Parties") are not responsible or liable for any accident, injury, harm or other loss that might occur to me resulting in any way from, or in connection with, the use of the Association's Property for the Recreational Activity, except such as may be caused by the sole gross negligence or willful misconduct of the Association.  I hereby forever and fully remise, release, acquit, discharge and indemnify the Released Parties of and from any and all causes of action, claims, suits, counterclaims, damages and demands of whatever kind and nature, known or unknown, anticipated or unanticipated, related to use of the Association Property, as may be caused by the sole gross negligence or willful misconduct of the Association.

5.	I understand and acknowledge that the instructor or volunteer coach for the Recreational Activity is not an employee, agent or contractor of the Association and that the Association, its members, officers, directors, employees and agents are not in any way whatsoever responsible for the instructor or volunteer coach’s action or inaction or any claims, costs, causes of action, or liabilities for personal injury or death, or damage to or destruction of property, arising from such instructor or volunteer’s action or inaction. 

6.	I further consent to the rendering of emergency first aid and other medical procedures to me which at the time or injury or illness seems reasonably advisable.  I further will be responsible for payment of any such medical procedures. 

7.	I understand that the Association and its members, agents and employees may photograph and record me and any of my family members, employees, agents, servants, guests, invitees, and any other person that is in any way related to the above activity and/or use of the Association property during the Recreational Activity.  I grant the Association and its agents the following rights: permission to use, publish, modify or alter the images for any lawful purpose whatsoever, including but not limited to promoting Legacy Park and the Association Property, on the Legacy Park website and on the Internet, and worldwide in perpetuity for the purposes sated above, without payment or any other consideration; and 
8.	I acknowledge that I have read this form carefully before signing.

 I am aware that participation includes potential risks to me of exposure directly or indirectly arising out of, contributed to or by, or resulting from an outbreak of any communicable disease, including, but not limited to, the virus “severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)”, which is responsible for Coronavirus Disease (COVID-19), and/or any mutation or variation thereof.
